In order for our physicians to provide you with the best care, it is important we gather information about your current and past medical history, medications, and your current urologic problem.

All new patients are required to complete our new patient registration forms. (These Forms are available on our Downloadable Forms page.) Completed forms can be returned by fax, or postal service. You may also bring the completed packet with you to your appointment. Please note: If you chose to mail your packet please allow at least three days or more for delivery. If you mail your forms, keep in mind you may have to fill them out again if we haven’t received them by the time of your appointment.

Once you establish care with us, we will update your information yearly to make sure our records are current. Our staff will update your medications and conditions with you at each visit before you see your provider.
